I, , hereby acknowledge and consent to participate in the Goddess Workshop Lipstick-Making Workshop. By signing this Client Consent & Release Form, I understand and agree to the following:

Use of Hot Wax and Open Flame: I am aware that during the workshop, hot wax and open flames will be used in the lipstick-making process. I understand the potential risks associated with working with these materials and will exercise caution and follow safety instructions provided by the workshop facilitators.

Potential for Burns: I acknowledge that some equipment used in the workshop may become very hot and could pose a risk of burning the skin. I agree to handle all equipment with care and to seek immediate assistance if I sustain any injuries during the workshop.

I understand that the lipstick-making ingredients may include tree nuts, beeswax, and mica color. I confirm that I do not have any known allergies or sensitivities to these ingredients. I acknowledge that I have been informed of the ingredients used and will take responsibility for informing the workshop organisers of any allergies or sensitivities I may have.

I hereby release Goddess Workshop and its organisers, instructors, and affiliates from any liability, claims, demands, actions, causes of action, costs, and expenses, including but not limited to medical expenses and attorney fees, that may arise as a result of any skin complications, allergic reactions, or illness caused by the ingredients used during the workshop.
